New patients can make an online appointment and fill out all the required paper work prior to their appointment. Patients should plan to arrive 15 minutes prior to the start of the appointment to complete the forms.

UCOK accepts most insurance plans. If you have a co-pay, please be prepared to pay at the time of the visit. Please be advised that patients are responsible for verifying if our providers are in-network and the co-pay amount. All patients will be seen by a medical professional at the time of their appointment.

We accept most health insurance plans and can bill the insurance companies directly. Just bring your primary insurance card, secondary insurance card (if you have one) and the policyholder’s name, date of birth and social security number. Insured patients are expected to pay their outlined urgent care co-payment.
